This Consent and Testing Disclaimer applies to services offered through nycdnatesting.nyc, which is operated by NYC DNA TESTING OF THE BRONX LLC (“Company,” “we,” “us,” or “our”).

This page is intended to provide general information regarding consent, authorization, specimen collection, and testing procedures. It should be read together with our Privacy Policy, Terms and Conditions, Refund and Cancellation Policy, and Genetic Data Privacy Notice.

1. Website Information Only

The content on this Website is provided for general informational purposes only.

Information on this Website does not replace formal consent forms, specimen collection requirements, identification procedures, laboratory documentation, legal requirements, medical advice, or legal advice.

Use of this Website, submission of a form, or communication with us does not by itself create a testing agreement or constitute formal consent to DNA testing.

Before any DNA testing service is performed, participants may be required to complete separate consent and authorization documents.

Depending on the service, required documents may include:

  • consent forms,
  • authorization forms,
  • government-issued identification,
  • parent or guardian authorization,
  • chain-of-custody or similar documentation,
  • laboratory forms,
  • specimen collection documentation, and
  • any other records or documents required by the laboratory or applicable authorities.

We reserve the right to delay, refuse, or cancel services if required consent or documentation is not completed.

3. Identification Requirements

All participants may be required to provide valid identification and other requested information before specimen collection or release of results.

We reserve the right to verify the identity of participants, parents, legal guardians, authorized representatives, and any person requesting access to testing-related records or results.

Failure to provide requested identification or documentation may result in delays, cancellation, refusal of service, or inability to release results.

4. Minors

If a requested service involves a minor, a parent or legal guardian may be required to authorize the testing and provide documentation showing authority to consent.

We reserve the right to refuse or delay testing involving a minor if proper authorization, documentation, or identification is not provided.

5. Testing Procedures

All DNA testing services arranged through NYC DNA TESTING OF THE BRONX LLC are subject to our required collection, identification, consent, documentation, and specimen-handling procedures.

Participants are responsible for complying with all required procedures, instructions, and documentation requests.

We reserve the right to refuse, reschedule, delay, or cancel testing if required procedures are not followed.

6. Specimen Collection and Handling

Specimen collection must be completed in accordance with our procedures, laboratory requirements, and any applicable documentation requirements.

We reserve the right to determine whether a specimen collection is acceptable for submission, whether recollection is necessary, and whether additional documentation or identification is required.

We are not responsible for delays, rejection of a specimen, recollection requirements, or inability to proceed where collection requirements are not satisfied.

7. No Guarantee of Third-Party Acceptance

We do not guarantee that any DNA test result will be accepted by any court, embassy, government agency, immigration authority, employer, or other third party.

Acceptance of any result may depend on that third party’s own rules, procedures, standards, documentation requirements, and review process.

8. Accuracy of Information Provided

Clients and participants are responsible for providing accurate, complete, and truthful information when requesting services, completing forms, providing documentation, or communicating with us.

Incomplete, inaccurate, or misleading information may result in delays, additional fees, cancellation, invalid documentation, refusal of service, or inability to release results.

9. Communications

By submitting a form, requesting an appointment, or otherwise contacting us, you agree that we may communicate with you regarding your inquiry, scheduling, documentation, appointment status, requested services, and related matters by phone, email, and, where applicable, text message.

Standard message and data rates may apply.

Services and information provided through this Website are not a substitute for medical advice, legal advice, or other professional advice.

You should consult a qualified professional regarding any medical, legal, or personal decision related to DNA testing, pregnancy, parentage, family matters, immigration matters, or any other legal or health-related issue.

11. Results and Release of Information

Results and testing-related information may be released only in accordance with our procedures, applicable documentation requirements, and applicable law.

We may require identity verification, written authorization, or additional documentation before releasing results or related records.

We reserve the right to withhold release where required documentation, payment, identification, or authorization has not been completed.

12. No Testing Agreement Until Accepted

Submission of a form, call, text, email, or appointment request does not guarantee service and does not create a binding testing agreement until the request has been reviewed and accepted by us and all applicable requirements have been satisfied.

13. Changes to This Disclaimer

We may update or modify this Consent and Testing Disclaimer at any time by posting a revised version on this page.

Any updated version will become effective as of the posted “Last Updated” date.
